Coordinated Care When You Need a Specialist

Medical & Surgical Specialist Referrals

Medical & Surgical Specialist Referrals

At Caceres Medical Group, we believe that excellent medical care often requires teamwork. Many patients can be treated directly in our office, but some conditions require evaluation or treatment by a medical or surgical specialist.
When a specialist is needed, our goal is to help guide the referral process in an organized, timely, and medically appropriate way. We collaborate with patients, families, insurance plans, hospitals, imaging centers, laboratories, and specialist offices to help coordinate care.
Our priority is simple: to make sure you receive the right care, from the right specialist, at the right time

Specialists We May Refer To

Depending on your medical needs, we may refer patients to:

Our Referral Coordination Process

At Caceres Medical Group, we do more than simply give you a name and phone number. When appropriate, our team helps with:
This coordination helps avoid delays, duplication of testing, confusion, and unnecessary emergency visits.

Why Referral Coordination Matters

Many patients have more than one medical condition. A senior patient, for example, may have diabetes, hypertension, heart disease, kidney disease, arthritis, and memory problems at the same time. Without good coordination, care can become confusing and fragmented.
Our role is to help keep the “big picture” organized.
We help patients understand:

Medical Referrals and Surgical Referrals

A surgical referral does not always mean surgery will be performed. It means the patient needs a surgeon’s professional evaluation to determine the best treatment option. When surgery is being considered, our office may also help with medical or cardiac clearance when appropriate, including review of risk factors, medications, EKGs, blood tests, and cardiovascular status.

Supporting Patients and Families

We understand that seeing a specialist can sometimes feel stressful. Patients may worry about the diagnosis, insurance approval, transportation, language barriers, or what to expect at the visit. Our team does its best to explain the purpose of the referral clearly and help patients prepare. We encourage patients to bring their medication list, insurance information, recent test results, and any questions they may have. Family members and caregivers are also welcome to participate when appropriate, especially for senior patients or patients with complex medical needs.

Our Commitment

At Caceres Medical Group, our commitment is to provide comprehensive care and coordinate with trusted medical and surgical specialists when your condition requires additional expertise. We remain involved in your care before, during, and after the referral so that your medical treatment stays organized, safe, and focused on your health.
